How to Enter

Visit www.monsterbymistake.com from 12:00:01 am EST Saturday, November 16, 2002 to 11:59:59 pm EDST Thursday, May 15 2003 (the "Contest Period"). Complete the online entry form by filling in your name, address (with postal code), age, telephone number, title of the Monster By Mistake book you are reviewing and your book review in the space provided. Then submit the entry form by clicking on the "Enter" button. Maximum one (1) entry per person per day.

A fully completed and submitted online entry form, including parental consent if the entrant is under the age of 18, will constitute one (1) entry into the contest. Entries must be received by Monster by Mistake Enterprises Ltd. during the Contest Period. Proof of sending will not be deemed to be proof of receipt.

Prizes and Draw

A total of Six (6) Grand Prizes are available to be won by random draw. Each Grand Prize consists of a full set of Eight (8) Monster By Mistake chapter books, a full set of Four (4) Monster by Mistake early reader books, a copy of volume one of the Monster by Mistake VHS Home Video collection, Ten (10) Monster by Mistake temporary tattoos and One (1) Personalized Monster By Mistake Wall Poster. Approximate retail value is Eighty Dollars ($80).

A random draw from all eligible entries will take place on the 15th day of each month at approximately 2:00 PM EST in Toronto, Ontario. At each draw, a Grand Prize will be awarded to the author of one chapter or early reader book review. Chances of winning are dependent upon the number of eligible entries received. Maximum one (1) random draw prize per household. Selected entrants will be notified by mail. In order to win a prize, selected entrants will be required to correctly answer, without assistance of any kind whether mechanical or otherwise, a mathematical skill testing question to be administered by phone. If the selected entrant cannot be contacted and the skill testing question administered after three (3) attempts within fourteen (14) days of selection, the prize will be forfeited and may be awarded to an alternate entrant. Selected entrants will also be required to sign and return a Declaration and Release Form which, among other things, releases Monster By Mistake Enterprises Ltd. and Stewart House Publishing Inc. (the "Sponsors") from all liability. If a selected entrant is under the age of majority in his/her province of residence, his/her parent or legal guardian must sign the Declaration and Release documents and accept the prize on the selected entrant's behalf. All documents must be returned within 14 days of the date indicated on the prize notification or the prize may be forfeited and awarded to an alternate entrant. Prizes must be accepted as awarded with no substitutions, whether in cash or otherwise, are non-transferable and will be delivered to the confirmed winners' address as shown on their entry. Sponsors reserve the right to substitute a prize of equal or greater value if for any reason a prize cannot be awarded as described. By accepting a prize, each winner agrees to the use of his/her name, city of residence, photograph and/or other likeness in any related publicity, including presence on Sponsors' website, without further compensation or permission, worldwide and in perpetuity. Sponsors make no representation or warranty that any identification, videotape, photograph and/or likeness of any winners will be broadcast or used otherwise in any manner.


Eligibility

The contest is open to all individual residents of Canada, excluding Quebec, who are located in Canada at time of entry, except employees, representatives, and agents of either of the Sponsors, their respective affiliates, related companies, advertising and promotional agencies and the independent judging organization and members of the household of such employees, representatives and agents. By entering, contestants agree to abide by the contest rules and all decisions of the contest judging organizations, which shall be final and binding on all entrants in all matters relating to this contest. This contest is subject to all Federal, Provincial, and Municipal laws and regulations. Contest is void where prohibited by law. In order to be eligible to win a prize, entrants must comply with these Contest Rules.

Miscellaneous

All entries become property of Monster By Mistake Enterprises Ltd. upon receipt and will not be returned and no correspondence will be entered into except with selected entrants. No responsibility will be taken for any failure of the website during the promotion or for any problems or technical malfunction of a telephone or broadcast network or lines, computer on-line systems, servers, access providers, computer equipment, software, failure of any e-mail, on-line, or Internet entry to be received by Monster By Mistake Enterprises Ltd. on account of technical problems or traffic congestion on the Internet or at any website, or any combination thereof that may affect an individual's ability to participate in the contest, or for any injury or damage to any person or to an entrant's or any other person's computer or property related to or resulting from playing or downloading any material in the promotion. Sponsors reserve the right, in their sole discretion, and without notice to cancel, suspend or modify this contest should a virus, bug or any other cause of any nature beyond the reasonable control of Sponsors corrupt the security or proper administration of the contest. Any attempt to deliberately damage any website or to undermine the legitimate operation of this promotion is a violation of criminal and civil laws, and should such an attempt be made, Sponsors reserve the right to seek remedies and damages to the fullest extent permitted by law, including criminal prosecution. Entries are subject to verification and will be declared invalid if they are illegible, incomplete, mechanically reproduced, mutilated, forged, falsified, altered or tampered with in any way. In the event of a dispute as to the identity of an entrant, the entry will be deemed to be submitted by the established holder of the Internet account associated with the entry.

By entering, participants agree that Sponsors shall not be responsible or liable for any losses or injuries of any kind resulting from participating in this contest or acceptance, possession and/or use or misuse of any prize and agree to release, discharge and hold harmless Sponsors and their respective officers, directors, parent companies, subsidiaries, affiliates, related companies, advertising and promotional agencies and prize suppliers from all claims or damages arising out of participant's participation in the contest and/or acceptance, use or misuse of any prize.

An entrant under the age of 18 must have the consent of his/her parent or legal guardian to submit their personal information in order to enter the contest. The parent or legal guardian must complete all the required fields on the relevant portion of the web entry form, including his/her name and address. If a parent or legal guardian does not provide consent, the entry will be disqualified. All information obtained for this contest will be used only for the purposes of conducting the contest and will not be shared with any other party.

A list of the best book reviews, including the contest winners will be posted and updated at the www.monsterbymistake.com website beginning December 1, 2002.
